Lymph Node Test Doesn't Improve Breast Cancer Care, Study Finds (HealthDay)
TUESDAY, July 26 (HealthDay News) -- A lymph node-sparing test hailed as revolutionary for its conservative approach does not lead to longer survival times for women undergoing lumpectomies whose early-stage breast cancer has spread microscopically, a large, new study suggests.
Examining the medical records of more than 5,200 patients who underwent breast-conserving surgery for early, invasive breast cancer, researchers found that tiny cancer cells in the sentinel lymph node -- the first node to which malignant cells are likely to spread from a primary tumor -- detected with a diagnostic procedure called immunohistochemical (IHC) staining had no effect on overall survival.
The biopsy procedure known as sentinel lymph node (SLN) dissection has been praised for averting the removal of large numbers of armpit lymph nodes during breast cancer surgery, which can lead to a painful buildup of fluid called lymphedema.
"I think I'd have to say it was highly controversial whether these occult [hidden] metastases would be clinically relevant," said study author Dr. Armando Giuliano, chief of surgical oncology at Cedars-Sinai Medical Center in Los Angeles. "If it's not going to affect mortality, it shouldn't affect treatment."
The study is published in the July 27 issue of the Journal of the American Medical Association.
The observational study included data from women included in the American College of Surgeons Oncology Group trial at 126 sites from May 1999 to May 2003, and all patients were followed until April 2010. Results were blinded to treating physicians to avoid the bias of overtreatment, Giuliano said.
At a midpoint follow-up of 6.3 years, 435 women had died and 376 experienced recurrence of their cancer. Based on IHC staining, five-year overall survival rates of those whose samples tested positive for node involvement were 95.1 percent, compared to 95.7 percent for those whose SLN biopsies tested negative. Corresponding five-year rates of disease-free survival were 90.4 percent and 92.2 percent, respectively.
Giuliano said the research, when adopted clinically, can save patients several hundred dollars or more in unnecessary tests. Women with microscopic metastases in their SLNs can also be spared from certain more aggressive treatments that were thought to increase their survival rates, he said.
Dr. Lora Weiselberg, chief of breast cancer service at the Monter Cancer Center of North Shore-LIJ Health System in Lake Success, N.Y., said many physicians have questioned the significance of the SLN biopsies evaluated in the study.
"In certain cases . . . we would want to do them anyway, but otherwise I think this is very strong evidence," Weiselberg said. "I think more and more pathology laboratories are going to go in that direction [of fewer tests] because adding another costly test, if it's not going to help the patient, is unnecessary."

The company's digital revenue, which comes partly from games for smartphones and tablets, rose 11 percent to $209 million.

Her father, budding lounge singer Mitch Winehouse, was in New York at the time of his daughter's death and flew back to England to join Amy's mother, Janis, and brother, Alex, in mourning the "Rehab" star.

Even those who'd never met Winehouse attested to the breadth of her talent, including Lady Gaga, who posted the following: "Amy changed pop music forever. I remember knowing there was hope, and feeling not alone because of her. She lived jazz, she lived the blues."
One of the most touching and personal tributes to Amy Winehouse came from actor Russell Brand, who also famously struggled with substance abuse before finding sobriety in 2003. Brand wrote a lengthy post on Saturday in which he touched on the dangers of addiction and sadness over an artist he called a genius.
"Whether this tragedy was preventable or not is now irrelevant. It is not preventable today," he wrote. "We have lost a beautiful and talented woman to this disease."
An autopsy is scheduled for Monday in London and police have said the death is being treated as "unexplained" at this point, with no foul play suspected.
After releasing the promising debut Frank in 2003, Winehouse rose to international stardom on the back of 2006's smash Back to Black. The album won her five Grammys and international acclaim as a groundbreaking artist who mixed the finger-snapping girl group sound of the 1960s with classic jazz crooning and a decidedly 21st-century attitude and swagger.
Fame appeared to only exacerbate whatever personal demons Winehouse was struggling with, as her travails with drug and alcohol addiction, messy personal life and botched concerts quickly overtook talk of her singing prowess. Though she had reportedly been at work on-and-off on a follow-up to Black for several years, at press time a spokesperson for the singer could not be reached for comment on whether Winehouse had completed those sessions and if they produced any music that is slated for release.

The American Consumer Institute - ? Christenson Blogs: Online ...
Last year?s Combating Online Infringement and Counterfeits Act (COICA) was widely chided as a massive overreach of executive power. ?Essentially, the bill would allow federal enforcement agencies to force ISPs, Search Engines, Domain Name Registrars and online financial transaction companies, to stop recognizing certain websites that have been deemed ?rogue?: those involved in intellectual property theft or copyright infringement.
The bill made it past the Senate Judiciary Committee, but was put on hold. ?Ron Wyden (D-OR) vowed to make sure the bill never made it to the light of day. ??Deploying this statute to combat online copyright infringement seems almost like using a bunker-busting cluster bomb when what you really need is a precision-guided missile,? said Wyden. ??The collateral damage of this statute could be American innovation, American jobs, and a secure Internet.? ?After a legal and policy experts shot holes in the bill, it was laid to rest for the year.
Despite the clear message from Congress that executive overreach into the free and open Internet would not be tolerated, the past few months have seen a share of problems. ?On November 26, 2010 the Electronic Freedom Foundation reported:
?Over the past few days, the U.S. Justice Department, the Department of Homeland Security and nine U.S. Attorneys? Offices seized 82 domain names of websites they claim were engaged in the sale and distribution of counterfeit goods and illegal copyrighted works.
?Setting aside the due process concerns inherent in seizing any website without notice or appropriate recourse for the owner, it appears that the ?raid? has swept up several sites that are hardly in the business of willful copyright infringement.
The seizures set a new precedent, further stretching the bounds of U.S. executive power into the Internet. ?This was exactly the sort of power Wyden and others were afraid that COICA might increase.
On May 12 of this year, a bill eerily similar to COICA found its way to the Senate chambers, introduced again by senator Patrick Leahy (D-VT). ? And if renaming a piece of legislation could change its content, the ?Preventing Real Online Threats to Economic Creativity and Theft of Intellectual Property Act? or the ?PROTECT IP Act? would be a vast improvement over COICA; a no-brainer. ?Sadly, this bill is COICA on steroids.
If passed, The PROTECT IP Act, like its predecessor, would allow the Department of Justice to seek a court order against what are commonly called ?rogue websites? -websites purported to be engaging in copyright or intellectual property ?infringing activities.? ?
Rather than addressing the major systemic problems in the COICA bill, Leahy has ignored the advice of legal and policy experts and is hoping a more attractive name might be enough to help poor legislation slide through Congress.
The most obvious problem upon first read of the bill is that it fails to define ?rogue? website. ?The bill broadly defines such a site as one that has ?no significant use other than engaging in, enabling, or facilitating the:
reproduction, distribution, or public performance of copyrighted works, in complete or substantially complete form, in a manner that constitutes copyright infringement under section 501 of title 17, United States Code.
This ambiguity in what is considered a rogue website gives government officials an uncomfortable amount of leeway when shutting down websites.
If lawmakers and lawyers do identify a website as being rogue, the PROTECT IP Act would employ a strategy known as DNS filtering to block (or force third parties to block) the site?s DNS. ?This sort of filtering is problematic in a number of ways itself. ?So problematic, in fact, that some of the foremost scholars on Internet infrastructure took it upon themselves to produce an apolitical white paper addressing the concerns. ?Paul Vixie from Internet Systems Consortium, Danny McPherson from Verisign and others illustrate the following concerns:
- The DNS provisions would undermine the universality of domain names, which has been one of the key enablers of the innovation, economic growth, and improvements in communications and information access unleashed by the global Internet.
- Migration away from ISP-provided DNS servers would harm efforts that rely on DNS data to detect and mitigate security threats and improve network performance. Dependencies within the DNS would pose significant risk of collateral damage, with filtering of one domain potentially affecting users? ability to reach non-infringing Internet content.
- The site redirection envisioned in Section 3(d)(II)(A)(ii) is inconsistent with security extensions to the DNS that are known as DNSSEC. The U.S. Government and private industry have identified DNSSEC as a key part of a wider cyber security strategy, and many private, military, and governmental networks have invested in DNSSEC technologies.
- If implemented, this section of the PROTECT IP Act would weaken this important effort to improve Internet security. It would enshrine and institutionalize the very network manipulation that DNSSEC must fight in order to prevent cyberattacks and other malevolent behavior on the global Internet, thereby exposing networks and users to increased security and privacy risks.
?Consumers and innovators alike benefit from a well-crafted and robust protection of intellectual property. Peer-to-peer torrenting of illegally copied films, music, and software presents a unique threat to intellectual property that needs to be addressed. ?Unfortunately, the PROTECT IP Act sacrifices the free and open Internet, cybersecurity, and certain freedoms in its attempt to address the problem, making it simply the wrong answer for consumers.

You have seen the commercials and advertisements for free non profit grants, but can you really get non profit grants or is it just a big scam? Well, the answer is both. Stay with me for a minute and I will try to save you some time and give you some good resources at the bottom of the article.
If you do an online search of free grant applications, you are sure to come across tons of websites offering free government grants. Some of them are resource guides for free grant applications and some of them are for organizations that actually award non profit grants. Both of these types of sites have value.
Let?s start with sites that actually have non profit grants to award. This is where you need to be when you are actually applying for money. These sites will have specific requirements that you must meet in order to be eligible. Sometimes the money has to be used to start a business. Sometimes the money has to be spent on education. Sometimes you have to live in a certain area or belong to a certain demographic. The list of possible requirements is endless, but if you meet them you have a chance to collect the non profit grants they have available. The problem here is that you have to find each opportunity one at a time.
The second type of site will have a whole bunch of free grant applications for you to fill out. These sites are basically guides to a bunch of individual sites like we talked about above. These sites are usually membership sites that require you to pay a monthly fee or buy a book. If you are able to get hundreds or thousands of free grant applications it is probably worth the money because you have very little chance of finding all the grants that are out there on your own and if you did it would take you hundreds of hours of research just to find them. Getting a resource guide will save you a ton of time and time is money. The one thing you can do is take advantage of free trials. Basically what you do is sign up for a free trial and pay a small shipping and handling fee. At the end of the trial you have to pay for the service, but if you cancel before the end of the trial, you usually get to keep the guide. If nothing else, you have anywhere from a week to a month to find all the grants you can. The links at the bottom have a listing of some of the services that provide free grant applications.

Oregon rangers rescue man from fall at Crater Lake (AP)
GRANTS PASS, Ore. ? Rescue rangers going over the rim of the collapsed volcano that forms Crater Lake weren't sure whether the man crumpled against a tree 300 feet below was alive or dead.
They had rappelled 100 feet down a near-vertical ravine covered with snow and loose rock and were rigging a new set of ropes to a tree to go the rest of the way when the cries from above reached them.
Someone had seen the man move, and was telling him to stay still, rescuers were on the way, said Jason Ramsdell, ranger operations supervisor at Crater Lake National Park.
"Two things happen," when visitors ignore the signs and get too close to the edge of the caldera wall, said Ramsdell. "Either they don't fall that far and have minor injuries, or they fall very far and are dead.
"It was surprising he was moving and alive," said Ramsdell.
Crater Lake is Oregon's only national park and draws about 500,000 visitors a year. Most of them drive up to Rim Village, look out over the spectacular expanse of blue circled by sheer rock, and go on their way. Many others ignore the warning signs and venture past the rock wall intended to keep them safe.
People and things going over the edge are nothing new. Last September, an Ashland couple forgot to set the parking brake on their car and the vehicle rolled over the rim into the caldera with their dog inside. The dog escaped through the sun roof, before the car fell some 1,000 feet.
In February 2009, rescuers hauled up a Klamath Falls man who suffered only scrapes after sliding about 200 feet over the edge while trying to retrieve a friend's cellphone.
On Tuesday, park spokeswoman Marsha McCabe said 27-year-old Eric Brimlow of Syracuse, N.Y., was visiting the area with friends Monday morning, when he left the path and stairs leading down to the Sinnott Memorial Overlook, a spectacular rock balcony built into the side of the rock wall left after the volcano erupted and collapsed upon itself more than 7,000 years ago to form the lake.
Witnesses told rangers that Brimlow stepped off the path about halfway down, stepped on a rock wall and leaped across a 5-foot wide chasm where the snow had melted away. When he landed, the man apparently slipped and slid headfirst down the steep snow-covered slope.
As he slid, the snow turned to loose rock, he tumbled and rolled, went over a 50-foot cliff, and was saved from sure death by coming to rest against a tree, about 700 feet above the lake, said Ramsdell.
"For whatever reason that day, he was saved by landing where he did ? landing against that tree," Ramsdell said.
Some five hours later, a total of 49 park personnel joined in getting him to the top. Seriously injured and unconscious, the man was flown by Mercy Flights helicopter to Rogue Medical Center in Medford, where the hospital reported Tuesday he was in critical condition.
Nick Atkins, dining room manager at Crater Lake Lodge, was coming to work when he was passed by ambulances, and then saw about 50 onlookers watching park rangers setting up their ropes for the rescue.
"I just don't understand how anybody could lean far enough forward they could slip. But things happen," said Atkins.
A ranger from the overlook spotted the man against a tree, and Chief Ranger Pete Reinhardt tied off from a tree and rappelled 100 feet down a ravine. Ramsdell joined him and they rigged more ropes to another tree to move down and across the loose rock slope.
Reinhardt found the man unconscious but breathing, pinned against a tree at the base of a five-foot cliff. Reinhardt tied the man in and with ranger Christina Sheppard tried to hoist him to the top of the 5-foot cliff, where the ground was flat. But they needed help.
Biologist Mark Buktenica took over managing the ropes at the midpoint belay. Ramsdell and ranger Paul Schauer rappelled down where they rigged pulleys to a tree 20 feet above the man, and the four rangers pushed and pulled him up.
On flat ground, they bundled the man in the litter. Ramsdell and the rest scrambled back up the wall and Reinhardt went with the litter while others hauled on pulleys for the long climb to the top.
Coming out the way the man went in, the team put extension ladders across the snow chasm and slid the litter to the path.
"I am a rock climber and I would not begin to think about doing what the rangers did," Atkins said.
