Job Description The purpose of this position is to provide program oversight and case management for residents of the group home. Description of Job functions: 1. Oversee program operation and development. 2. Help review the appropriateness of admission of each child to the group home. 3. Participate in developing, reviewing and updating resident assessments and treatment planning. 4. Provide technical assistance to the resident care staff and agencies. 5. Periodically review and update group home policies and procedures. 6. Provide individual and group counseling. 7. Coordinate child welfare activities with local agencies, biological parents (when applicable), and foster parents. 8. Conduct weekly sessions with youth in placement. 9. Coordinate quarterly meetings of the treatment team. 10. Develop a treatment plan within 30 days of placement. 11. Responsible to help ensure that youth is enrolled and attending school. 12. Provide life skill training to clients. 13. Develop a individualized treatment plan (ITP) for each placement with goals during placement. 14. Work with staff to implement ITP. 15. Monitor the progress and/or lack of progress of the ITP. 16. Monitor cases to ensure medical, dental and psychological needs are met. 17. Provide 24 hour crisis intervention for children on caseload. 18. When appropriate maintain regular contact with biological parents and arrange interaction between child and family. 19. Accompany all children to court appointments and Administrative Reviews. 20. Develop aftercare plans for children leaving placement? ? Qualifications The qualifications for this position: 1. At least 21 years old 2. Have a 4-year college degree in social work, sociology, special education, psychology, counseling and guidance, criminal justice, or any other human services field as approved by the child welfare department. 3. 2 years supervised working experience in child welfare with individuals and families.? ? Requirements 1. The ability to work with a diverse population. 2. Knowledge of the child welfare system and knowledge of community resources. 3. Possess at time of hire a valid driver?s license with insurance. 4. Excellent written and oral communication skills. 5. Must be able to work independently and as part of a team.? ? We currently have two group homes located on the northwest side of milwaukee. Our group homes provide services to teens ages 12 - 17. We also provide on going services to youth ages 18 - 21 who are exiting our group homes and/or the foster care system. The group homes provide everything that a loving and stable family provides: love, praise, spiritual encouragement, clear expectations, education, daily living and life skills, enrichment experiences and 24 hour supervision in a safe and structured setting.
